Currently we are accepting submissions from:

February 2024 Cohort:

• Part 1, Case Studies deadline: Jan 6, 2025
• Part 2, Competency, Attendance Record (you are on this form now, scroll down to continue) deadline: Feb 17, 2025
• Course Platform Assessments deadline: Feb 10, 2024
• 6-month extended deadline: Sept 23, 2025

September 2023 Cohort: final 6-month extended deadline: Mar 31, 2025

May 2023 Cohort - final 6-month extended deadline: December 10, 2024
 

Submissions during the 6-month extension period:
 

• There is a $50 CAD late fee to submit your course completion requirements within the 6-month extension period. You can pay the $50 late fee here.
• No further extensions will be granted beyond the 6-month extension deadline. You have the option of repeating the training in subsequent years to complete any work that remained incomplete in previous years. The cost to repeat the training is $1000 CAD.

• There are two pages to this form. On page one, you must fill in your complete address, and hit the pink button to continue to page two. On page two you will submit your forms.
• You may save this form as a draft if you would like to take more time to submit.
• When you are ready to submit, please ensure you click the final Submit button at the bottom of page 2 by the deadline noted above.
• Please reach out to Gabriel at gabriel@compassionateinquiry.com once you have completed all course requirements during your 6-month extension period. Please note that it is your responsibility to notify CI of your course progress after the regular deadline. 

The information, files, and videos you submit are strictly confidential and will only be seen by the certification team (comprised of the CI Facilitators, and only if necessary, the Admin Team).
